A2M Therapy
Available at our Raynham, MA clinic
Alpha-2-Macroglobulin therapy targeting cartilage-degrading enzymes in painful joints.
A2M is a naturally occurring protein in your blood that captures and inactivates the enzymes that break down cartilage. Concentrating it and delivering it directly into a painful joint may slow degradation and create a healthier environment for repair.
In depth
What A2M Therapy actually does.
Alpha-2-Macroglobulin (A2M) is a large protein your body already makes to neutralize the destructive enzymes that erode cartilage — chief among them MMP-13, ADAMTS-4, ADAMTS-5, and cathepsins. In a healthy joint these enzymes stay in balance. In an arthritic or injured joint, they run unchecked and cartilage breakdown outpaces repair.
A2M therapy interrupts that cycle at the source. We draw a small amount of your blood in the office, concentrate the A2M through a specialized filtration process, and inject it under image guidance directly into the painful joint. Once inside the joint, A2M binds and inactivates the enzymes driving degradation, giving the joint an environment where healing can outrun destruction.
Because A2M is your own protein, there is no rejection risk and no medication side-effect profile to manage. It is a biologic solution to a mechanical problem — and for the right candidate, it can meaningfully change the trajectory of a painful joint.
How it works
A clear, step-by-step process.
- 1Blood draw in office
- 2Specialized concentration of A2M from your plasma
- 3Image-guided injection into the target joint
- 4Structured rehab and follow-up at 6 and 12 weeks
What it's used for
The specific problems A2M Therapy solves for our patients.
Mild-to-moderate osteoarthritis
Knees, hips, shoulders, ankles — joints where cartilage is thinning but not yet bone-on-bone in every compartment.
Post-traumatic joint pain
Old sports injuries or surgeries that left a joint chronically inflamed and stiff.
Persistent pain after cortisone or hyaluronic acid
When symptom-only injections have stopped working, A2M targets the underlying enzymatic damage.
Cartilage preservation
Patients who want to protect what cartilage they have left and delay or avoid joint replacement.
Why patients choose it
- Uses your own biology — no synthetic drugs, no immune reaction
- Neutralizes the enzymes that destroy cartilage, not just the pain signal
- Complementary to PRP, physical therapy, and decompression
- Same-day procedure with minimal downtime
- One injection is often enough; results build over 8–12 weeks
Evidence & mechanism
Published musculoskeletal research has identified A2M as one of the primary inhibitors of the enzymatic cascade behind cartilage breakdown. Peer-reviewed studies have shown reduced pain and improved function after intra-articular A2M in patients with early-to-mid osteoarthritis, and it is now widely used across regenerative sports medicine practices.
Who is a good candidate
You are likely a good A2M candidate if you have persistent joint pain from mild-to-moderate osteoarthritis, have imaging showing cartilage wear but not complete bone-on-bone collapse, and want to preserve the joint rather than mask symptoms. We confirm candidacy with a focused exam, review of imaging, and a conversation about your goals.
The joints we see hold up longer.
In our Raynham office, A2M patients most often report meaningful relief starting at 4–6 weeks, with continued improvement out to 3 months. The patients who do best combine the injection with the movement and metabolic work we build into every plan — that combination is why so many of our patients have delayed or avoided joint replacement for years.
“My orthopedic surgeon told me I needed a knee replacement. Two years later I'm still golfing, still walking my dog, and still nowhere near an OR.”
What to expect
The visit takes about 60–90 minutes. Most patients walk out the same day with mild soreness for 24–72 hours.
